Contained on this all-in-one 8.5 x 11" PDF, this form allows you to edit "Company"s Name" and "Employee Emergency Contact Form" at the top to whatever your preference is. The area underneath gives you lines for the employee to hand write their personal contact information. There is also 2 more areas for the employee to hand write their primary and secondary contacts in case of an emergency, followed by an area for additional information. They can then sign and date at the bottom. Although the very top header area can be edited on the computer, the remaining areas must be hand written. You can save your master file and then print as many as you like!
